Samenvatting | Beim Einsatz einer primären Endoprothese profitieren sowohl ältere als auch Patientinnen und Patienten im berufsfähigen Alter von einer Aufweitung der Indikationen für eine zementfreie Verankerung. Dabei ist eine hohe Primärstabilität des Implantates eine Grundvoraussetzung für die erfolgreiche Osseointegration und das Erreichen der Sekundärstabilität. In dieser Arbeit zeigte sich, dass die Verdichtung des implantatnahen Knochens mit Knochenpartikeln den Haftreibungskoeffizienten und damit die Primärstabilität des Knochen-Implantat-Interfaces erhöht. Die Partikel können während der Operation autolog aus reseziertem Knochen gewonnen werden. Der Einsatz einer Knochenmühle mit Abstimmung des Mahlgrades auf die individuelle Mikrostruktur des Knochens steigert die Primärstabilität zusätzlich.
When using a primary endoprosthesis, older patients as well as patients of working age benefit from a broadening of the indications for cementless fixation. High primary stability of the implant is a prerequisite for successful osseointegration and achievement of secondary stability. This work showed that densification of the periprosthetic bone with bone particles increases the static friction coefficient and thus the primary stability of the bone-implant interface. The particles can be autologously obtained from resected bone during surgery. The use of a bone mill and the adjustment of the grinding degree to the individual microstructure of the bone further increases the primary stability. |
